Community nursing services can be provided in your home by our experienced qualified nurses to help you lead an independent and healthy life. This type of care is suitable where a participant requires more clinical nursing support where we can assist in your transition from hospital to home or residential facility. We work directly with participants and their families to prevent disease, maintain health as well as assisting existing health concerns.

• General day to day medical health supportd e, wound care, oral/IV/IM medication administration
• Palliative care nursing assistance
• Assessments – continence, holistic care plans
• Complex disability care – PEG, Continence, ventilator, bowel care.
